Thank you for your advance reservation for Helgi Tomasson: A Celebration.

If you are the host of a table or purchased multiple tickets and you would like to host a dancer, ballet master, or member of our Honorary Artistic Committee, please submit your requests to Meg Sullivan at msullivan@sfballet.org. All requests will be honored in the order in which they are received.

The pleasure of your RSVP with guest information is requested by Wednesday, April 6. Guest information may be shared via email to Meg Sullivan at msullivan@sfballet.org.

EMAIL MEG GUEST INFORMATION

AT THE EVENT

At event check-in on April 24, 2022, per venue requirements, SF Ballet Safety Policy, and public health official recommendations, each guest will be required to bring:

1. Photo ID

2. Proof of full vaccination and booster against COVID-19

3. Proof of any negative COVID-19 test in addition to full vaccination (within 24 hours of event start time for antigen test; within 48 hours of event start time for PCR)

We will plan to send your complimentary at-home BinaxNOW antigen COVID-19 with test with eMed virtual proctoring service by mail to the address we have on file for you to fulfill the proof of negative COVID-19 test requirement. If you have elected to include artistic guests at your table, we can ensure they also receive tests.

Tests and detailed instructions with access to a complimentary test proctor will be mailed the week of April 14, 2022. You may also opt to take your own COVID-19 test (within 24 hours of event start time for antigen; within 48 hours of event start time for PCR); click here to view a map of COVID-19 testing locations. 4. A face mask

You will be required to wear a face mask when not actively eating or drinking during the event. SF Ballet requires all patrons to wear an N95, KN95, or KF94 face mask to enter the venue. SF Ballet will provide upon entry a KN95 mask or equivalent if you do not have one.

5. Performance ticket(s)

Your performance tickets will be mailed to you the week of April 14, 2022. If you are the host of your table or have purchased multiple tickets, you will receive all your tickets in the mail.
